Privacy & HIPAA
The information you submit constitutes Protected Health Information (PHI) under HIPAA. We are a HIPAA Covered Entity and handle all PHI with technical safeguards including AES-256 encryption at rest, TLS encryption in transit, audit logging, and 15-minute admin session timeouts. No PHI is stored in your browser. Social Security Numbers are not collected through this portal.

How We Use Your Information
Information is used solely for treatment, insurance billing, and health care operations at Davis Center for Oral and Maxillofacial Surgery. We do not sell your data or use it for marketing. We do not share PHI with third parties except as required for your care or by law, and only under executed Business Associate Agreements.

Your Rights
You have the right to access, correct, and request deletion of your records. You have full HIPAA rights including the right to request restrictions, an accounting of disclosures, and to file a complaint with HHS Office for Civil Rights.

These terms are governed by the laws of the State of Utah and applicable federal law including HIPAA.
